چکیده

Depression following a cerebrovascular accident is common, disabling, and treatable. However, the consequences of a stroke often render the clinical evaluation for depression misleading or difficult to interpret. These factors make a laboratory test for depression especially desirable in this population. We reviewed and evaluated the literature on the dexamethasone suppression test (DST) as a diagnostic tool for depression in stroke patients. Nine studies were identified. Our findings included (1) substantial variation in both methods and results, (2) a median specificity of 87%, and (3) a median sensitivity of 47%. We show that if these estimates of sensitivity and specificity are supported by future studies with improved methodology, then the DST may be clinically useful for the minority of stroke patients in whom a careful evaluation for depression remains inconclusive.

Hypercortisolism Early Versus Late After Stroke A 3-Year Longitudinal Study

Background and Purpose: Using the dexamethasone suppression test, we studied the suppressibility of the cortisol axis and its clinical determinants at various time points after stroke. A major aim was to examine the dexamethasone test as a diagnostic tool for the diagnosis of major depression in stroke patients. Methods: The dexamethasone suppression test, major depression, functional ability, ...

Hypercortisolism Revealed by the Dexamethasone Suppression Test With Acute Ischemic Stroke

Using the dexamethasone suppression test, we studied the activity of the hypothalamicpituitary-adrenal axis within the first week after onset in 62 patients with acute ischemic stroke. Compared with two control groups (one comprising 25 elderly patients with various acute medical disorders and the other comprising 33 80-year-old volunteers), stroke patients had higher postdexamethasone cortisol...

The Dexamethasone Supperssion Test in Endogenous Depression4

In 34 patients of endogenous depression and 30 normal controls, the dexamethasone suppression test (DST) had a sensitivity of 67.6%, a specificity of 93.3% and a positive predictive power of 92%, which compares favourably with other diagnostic tests in clinical medicine. DST non-suppressors were significantly more depressed as compared to DST suppressors, had a higher incidence of suicidal atte...
